Diabetes Prevention Across the Lifespan Webinar

This webinar will highlight the key takeaways from MHP Salud and HOP’s 2020-2021 article series: Diabetes Across the Lifespan. The facilitators will
share expertise on utilizing community health workers to improve health outcomes, demonstrate opportunities for partnering with other community
organizations, and showcase success stories from community health centers implementing innovative strategies around addressing diabetes.
